New Policy on Change of Name Application

ISC remains committed to better serving customers by ensuring data integrity and preventing fraud. One way we can fulfil this commitment while reducing customer inconvenience of form rejections is to ask for evidence of legal name changes.

Effective Oct. 5, 2015, evidence of legal name change will be required on Individual Change of Name applications. Applications submitted without the required evidence will no longer be accepted as of Oct. 5, 2015.

The requirement for evidence is one component of a larger initiative that started earlier this year to reduce form rejections and make changes to client accounts easier for customers. As part of this initiative, forms were separated and simplified, and clearer instructions were provided within the forms. At the same time, the online Individual Change of Name application form was updated to ask for evidence of name change. New forms have been available since July.

Some customers continue to use older versions of forms. Although we will still accept outdated forms, we strongly encourage customers to access the most up-to-date version of forms on the ISC website to reduce the chance of the forms being rejected. The updated forms have been simplified and include details about required information that will help customers avoid form rejections. Forms are available on ISC website.
